This is a silly one but make sure your extruder carriage hits both idler blocks in the front at once. If it's not your carriage is crooked and tnis can happen because the rods "bow in" when you insert them. For my newest iteration of carriages I drilled the holes out so I didn't have to hammer in the rods with a mallet. They aligned much better and the set screws hold them in place just fine.

I've also found it helps to over extrude your carriages a good bit for a tight fit. Given a good drive system though this isn't necessary tongue

During printing, a lot of what you're seeing could be explained by fluctuations in temp.

Why do you have a layer height of .38?
I use .2963 for every layer.
I know jagowilson uses a different first layer.
