Is your idler bearing centered with respect to groove on the drive gear? I found that I needed to move one of the washers around the idler bearing to the other side (so there's 3 on one side; one on the other) to center the bearing.
This happens to me whenever the nozzel is clogged.
Mine loosens its grip on the PEEK as it heats up and lets the opening get out of alignment. I have a kind of bar pushed on to the mounting knobs so the two sides of the mount can't spread apart.
Yeah there must be something else going on, there are so many of us using MK4 or previous versions without any issues. I haven't had any clogs since switching to MK4 but I did have to move my drive gear to align it with the MK4 opening.
I also had this issue till I moved the hobbed gear, also a failing hotend did this a couple of times.
