I know you don't want to hear this but you should replace the thermistor wires all the way to the board.  At least you'll eliminate that as the problem.

From one beard to another, don't do that. Keep that face sweater! wink

It sounds like the firmware took because you were able to reach a temp higher than stock for a short bit.
Stop uninstalling things. That is not where the problem lies.

If you went 6 minutes before failing, I raise you another dollar that it is a failed connection between the thermistor lines and the wires leading to the board where the crimp is.

Okay, I pulled everything, checked it for continuity, and then put it back.

No dice.

Could it be a power issue?  Do I need separate power supplies for the heater and the board maybe? I shouldn't but I'm just not sure what to try next.

You don't need a separate source for the E3D in any way. I am convinced this is a power issue whether it be from the board, PSU, surge protector/wall outlet....whatever it is, you are signal. This is why it works but then stops.

Do your LED's flicker while its printing?
How long will it heat when you're not printing?

Took a couple of days to cool my heals, went back in, pulled her apart again and retested the wires.  I changed the wires completely from the thermistor leads all the way to the board including a new connector at the board.

Same exact issue.  Heels no longer cool.

LEDs do not flicker when it is running, they are steady.  Have not tried heating it independently as I don't wish to clog it but it will not heat up again after the print shuts down until I let it sit for a while.

Have looked at the barrel connector that goes into the board.  Make sure it's secure maybe it is momentarily losing connection.

Did you forget the washer for the wires that hold the thermistor in place? It's important for proper strain relief.

Again, the help is much appreciated...

1) Got the washer in place for sure.  It is hard to get those wires under the screw so you almost have to have the washer.

2) I just now moved the barrel connector around (I assume you mean the one for the power) and there was no interruption to the LED or the fans so it appears pretty solid.

Also, again, the thing works perfectly for 4-6 minutes and then poof.  The prints I am getting for that period are real nice and tight.

...to see if it makes any difference at all, but I'm not so sure.

One last thing, does the standard power supply work?  Maybe I need a beefier power supply than the 12V/6A that was provided? Though I did check it and it wasn't over heating at all.

The PSU is not over powerful by any means.  If you have another to try you should.

Where do I get the config file from?  Just want to make sure I grab the right thing.

Also, I did check the heater voltage by removing the heater connection and turning on the extruder.  It measured at about 12volts as I recall.

So you re flashed your board.  Did you make the max temp changes in your configuration.h file before you flashed it?  If not there's your problem.
Try a print at 215 or just set it at that and see how long it stays there.

On second look, I know that the instructions say: #define TEMP_SENSOR_0 5 but wouldn't #define TEMP_SENSOR_0 1 work better? Possibly?

No, 5 is correct for the included thermistor.

I know you are frustrated, just hang in there.  What ever the problem is it will most likely be a simple fix.  Have you spent any time focused on the wires and connectors for the heat cartridge?

Yes, on the heater portion.  As above I tested the voltage coming out and that was okay.  I just now decided to try using the OHMmeter to test the wires from the heater and it pegged out - I have a crappy meter. 

Could that indicate a bad heater unit?

Sorry, it is an analog meter so pegged out means that the needle goes all the way over to the end no matter what setting I choose for Ohms.  I am now assuming that this would mean that either my meter sucks or the heating element is bad.

I'd try a digital multimeter if you can get your hands on one.

"over to the end" doesn't help us without knowing what the scale is on your meter. A photo would help.
