I have that same problem on prints and never been able to get an answer. Sometimes it reaches other times it does not. everone just keeps going on about adjusting the extruder but its perfect.

Your first problem is backlash - you need to fix your belts, probably y-axis. That's why your holes aren't round.

Backlash! I made an indicator holder last night. Will check and adjust after work today.

Played with the belts today and got it after an hour or so. Thanks for the insight on backlash!
