Found the E3D hot end cooling fan dangling by its wires and the extruder clicking away..

E3D Fan duct cracked in half.

You can clearly see where the layer fan failed. Just shows the difference a fan makes with pla !
Which fan duct were you using ?

I don't think it was the layer fan but it was the fan on the heat sink.  but where you using a printed fan duct or the one that came with the e3d?

I think you're right about it being the extruder fan, after reading the op again.
That would probably still cause the poor quality in the top of the print though.
Lack of cooling would cause poor extrusion.

The OEM E3D fan duct from kit cracked at the thin area where it was cut to fit.

Still had a prob. Superglued duct together and fan, but the extruder still skipped. 

Disassemble hotend.

From the heat break not getting any cooling from the fan,
filament pressured and pushed the PTFE tube up the heat break, the length of the thick part of the stuck filament, 10mm.
Then the jamming began.

You might want to torch it out and then replace the ptfe tube you most likely over heated that and melted it so you need to replace the portion that was damaged

Its fine.

Another prob I had in the beginning was the PTFE tube at the extruder was too close to the hobbed gears.
It was chewing up little pieces of the tube, going down the tube to the nozzle and clogging.
PTFE has a high melt. Was a mess to clean up.

Im re-thinking designing stuff for print. The failed print was a 5x5x3 4sided screw enclosure w/ bottom. Lid is separate.

Re-designed to a clam shell enclosure. 2 halfs at 3:30 ea. About the same amount of total time, but less time loss due to a failure.
