I adjusted the vref up a wee bit and not the extruder runs MUCH smoother without the mentioned "pulsing" noise, and the lines are sharp as a razor blade! Are you using the stock carriage that came with the printer? If so, it will push out the E3D at a very slight angle.

Is there a chance your nozzle isn't straight?
