I had similar issues, but not quite as bad as yours. From what I could find it was suggested to re-check the belt tension.
Easy in principle, however annoying because the grub screw on the pulleys was enough to burr the shaft making the pulleys not self home as easy as shown in the youtube vid. I physically had to push on them to free them up enough to align. But still I worked through it. I'd assume it helped (not as much as I would have thought).

A few weeks later I noticed the extruder wire bundle bumping into and bending the left Y axis belt. Kinda explained the slap noise I would sometimes hear.
I printed this and surprise surprise, holes now turn out better.

http://www.thingiverse.com/thing:94675

So I've managed to improve my print quality quite a lot already, but there's a few issues I'm not quite sure how to work out.

What I've done so far:

Extrusion stepper calibrated
Flow rate calibrated
Z wobble/backlash fixed
Bed is level
Z screw is set

I have no issues with the plastic curling up or not sticking. I use glass and hair spray, it's working quite well.

I'm printing at 0.3mm layers.

I've attached a few pics of a test plate I printed up, can anyone tell me how to fix these problems?

Please pardon the dirty fingernails, I work on a dirty dock all night moving freight. It doesn't come off easily.
