The stock plastic extruder arm on Ender 3s is notorious for developing cracks, often underneath where it's hard to see. This reduces the tension on the filament, so the gear can't grip it properly to push it. Carefully remove the extruder arm and inspect it thoroughly for any cracks. If it's cracked, replace it. An all-metal extruder upgrade is a very common and recommended fix for this.
