I don't know that it matters but it looks like the difference is which side of the drive gear the filament passes through. Probably more important for dual extruders where the motors would be side by side. A left and right option would allow orientation of the tension arms so the adjusters don't interfere with one another. Just my best guess, should probably reach out to the company and verify.
