Just edit the slicing app's start/end gcode with the info in the link above to enable z-offset ('auto-leveling') in 3rd party slicers.
When I entered your code in Repetier Host from Solidoodle it started printing with both extruder and bed cold.

I didn't change much in the Cura settings, except to completely remove the G-code startup code it inserts.

You could probably print some kind of clamp to keep the top open.
But in practice, heat build up wasn't an issue with the lid partially open and resting on the front door. I did several 1 hour prints back to back with no issues caused by head.

Be careful with printing with the lid entirely open. As you use up your filament and make the spool lighter, it becomes much more likely for the lid to slam shut. You can still rest the lid on top of the printer's front door to get a good amount of extra airflow, but without the risk of the lid falling. My lid stayed open initially but now it slams shut for every print if I forget to put it down.

]]>Switching to Cura, I ran into pillowing on top surfaces and a few other issues, so went back to Slic3r. Didn't put much time into it. Glad it's working well for you. Have you conquered PLA yet?
Haven't even attempted PLA yet. To be honest, I'm not that fussed. My understanding is that it starts to break down after a few months. I don't want that. In addition, I just don't think I want to have to go through this amount of pain again!
